Patient Care Coordinator


AmeriWound is seeking a Patient Care Coordinator that will possess ambition and seeks new tasks on a regular basis. This position serves as a member of the Patient Care Coordination Department, whose responsibility is to support the systems and operations of specialty provider services. Workflows supported by the department include but are not limited to: patient registration, patient eligibility, insurance screening, patient appointment scheduling, provider scheduling, provider documentation and consent audits, and resource utilization. Under the direction of the Patient Care Coordinator Manager, the Patient Care Coordinator triages all requests received, verifies member eligibility and creates all events in Electronic Medical Record.



Hours: 10am to 7pm (subject to change)

Compensation:  Competitive Hourly Rate


  • Provide high level of customer service to Providers and Client-LTC Facilities via phone, email, and in person
  • Scheduling and maintenance of Provider scheduling
  • Patient Registration and Patient Appointment Scheduling and/or changes
  • Manage incoming and outgoing faxes, face-sheets, and consents
  • Verify primary and/secondary insurance, initiate pre-certification for managed care patients and coordinate with Revenue Specialist
  • Coordinate coding procedures with Billing Specialist
  • Enter Clinical information in Electronic Healthcare Record (EHR)
  • Complete Audit Tracker and submit to Supervisor/Designee weekly.
  • Patient Medical Record maintenance, matching consents to face-sheets, filing/organizing by facility
  • Complete managed care authorization and referrals
  • Under the direction of the Patient Care Coordinator Supervisor, the Patient Care Coordinator triages all requests received, verifies member eligibility, ascertains benefit coverage and creates all events in EHR.
  • Attends weekly staff meetings and upcoming launch meetings with Supervisor and designated Area Manager(s) and AmeriWound Chief Operating Office (COO)
